Body dysmorphic disorder is a psychiatric disorder that is characterized by mental preoccupation with imagined defects in physical appearance or distorted perception of one's body image and experiencing negative emotions about appearance. the purpose of this study was to compare the effectiveness of paradox therapy (PTC) and cognitive-behavioral therapy on negative emotions of women with body dysmorphic disorder. the current research design was experimental with pre-test and post-test and follow-up with control group. The statistical population consists of all women suffering from dysmorphic of the body applying for cosmetic surgery who referred to the beauty clinics of Tehran in October and December of 2023. 45 patients were selected in a targeted way and randomly assigned to two experimental groups and one control group (15 people in each group). Questionnaires for metacognition assessment of body dysmorphic (Rabiei et al, 2011) and negative emotions (Loyband and Loyband, 1995) were used to collect information. Participants attended paradox therapy sessions 8 sessions) and cognitive-behavioral therapy (8 sessions). Data were analyzed using repeated measures ANOVA with SPSS version 22. The findings indicated that both treatments had a significant effect on reducing stress, anxiety, and depression, and the interaction between time and group was also significant (P ≤ 0.01), indicating differences in the pattern of change in negative emotions among the groups. The follow-up Dunnett’s test confirmed a significant reduction in anxiety, stress, and depression in both treatment groups compared to the control group (P ≤ 0.01), with paradoxical therapy demonstrating a stronger effect in reducing negative emotions. Given the effectiveness of paradoxical therapy, it is recommended that special attention be given to the techniques of this method for reducing negative emotions in women with body dysmorphic disorder.
